You will need to bring with you:
	Registration Form-Filled out completely
	Rules Form-Initialed & signed by both parent and child
	Medical and Consent Form-Filled out completely & signed
	Hold Harmless Form-Signed & dated
	Full Registration Fee - $80.00 (non-refundable)
	Multi-Child Discount (click to see more) 2 Children $150 3 Children $220 * Full LATE Registration Fee - $90.00 (AFTER November 1)
	Copy of Birth Certificate-Copy only
	Parents & child must attend orientation & Read & Sign Constitution -Parents Meeting/1st Day of Practice
